Transitional Services

When discharge approaches, the primary counselor will manage the development of a Post Placement Plan. A rules/expectation list must be included in the plan. The post placement plan form will be used as a guide. When a resident is close to accomplishing their placement goals, as indicated by their skill level in the program and goal accomplishment, a discharge site will be decided upon with input from the family and case worker.

Transition into the post-placement site should include visits of increasing length with family counseling appointments during the visits, if needed. If a foster home is determined as the post-placement site, attempts can be made to set up family counseling in coordination with visits to the foster home.
